Are Medicare premiums deducted from Social Security?

For most people receiving both, yes. Part B — and usually Part D — is taken out of the monthly Social Security payment automatically.

Once you are drawing Social Security and enrolled in Medicare, the Part B premium is withheld from your monthly benefit before it reaches your bank account. If you have a Part D drug plan or a Medicare Advantage plan with a premium, you can normally choose to have those withheld too, or pay the plan directly.

If you are enrolled in Medicare but have not yet claimed Social Security, there is nothing to deduct from, so Medicare bills you directly — quarterly by default, or monthly if you set up an automatic payment. People who delay their Social Security claim past 65 are often surprised by that first invoice.
