Original Medicare generally does not pay for care outside the United States, with a short list of narrow exceptions. Some Medigap plans include a limited foreign travel emergency benefit with a lifetime cap, and some Advantage plans include worldwide emergency cover — but 'emergency' is doing a lot of work in that sentence.

Generally no. Original Medicare pays for care outside the United States only in a few narrow situations. Certain Medigap plans add a limited foreign travel emergency benefit with a lifetime maximum, and some Advantage plans include worldwide emergency cover.

No. Cancellation cover protects the money you spent on the trip; travel medical protects you when you are treated abroad. They are often sold together and are worth separating in your head.
