Networks, in one page


An HMO keeps care inside a defined network and usually routes it through a primary care doctor. An EPO keeps the network but drops the referral. A PPO keeps both the freedom and some out-of-network cover, and charges for it.

For a household whose doctors sit comfortably inside one network, the PPO premium buys very little. For a household split between two hospital systems, or one that travels for months at a time, it can be the only plan that works.
